Carpool Form - If you participate in a carpool, please fill out this form and return it to Susan or Lindsay

Special Education Needs Form - If your child has special educational or emotional needs, please fill out this form and return it to the Education Director.

Brit Kehillah-Permission Form - This form includes a Brit Kehillah, as well as permission for Photo Use and Field Trips.  We ask all parents and students to read, discuss, and commit to the expectations/behaviors stated in the Brit Kehillah.  Please sign and return this Brit, or covenant, along with the photo and field trip permission form, to Susan or Lindsay.

Youth Kehillah Registration Form - If you would like your child to participate in Youth Kehillah, held the first Friday of each month, please return this form, along with a suggested donation of $5, to Susan or Lindsay.
